The Opportunity 

Yerin Eleanor Duncan is looking for a suitably qualified Cancer Navigator to join our Integrated Team Care, Chronic Disease Program. The Cancer Navigator will provide preventative and ongoing education regarding screening, treatment and support, and care coordination to patients diagnosed with cancer living on Darkinjung Country.

The successful candidate can meet all essential criteria:

  • Minimum Certificate IV in Community Services
  • Experience working in the health industry

Selection Criteria Questions

  1. Please give testimony to your Aboriginal identity and explain your connection to Darkinjung or the community you are from including your thoughts on how this relates to this role. 
  2. Outline your experience providing effective and efficient care coordination to patients throughout transition of care periods. 
  3. Provide examples of your experience in creating and delivering education programs for prevention, new diagnosis and ongoing care availability and supports. 
  4. Outline your experience liaising with external partners including General Practitioners, Nurses, Allied Health Providers, Specialists and Local Hospitals.
  5. Describe your understanding of trauma informed care and continuity of care.
  6. Outline your verbal and written communication skills, organisational skills and computer literacy.
