About the role

The Community Mental Health and Wellbeing Service (CMHWS) is a new initiative to meet the increased demand for mental health services as a result of the COVID-19 pandemic. The service provides wellbeing supports tailored to a person’s individual needs from early intervention to specialist referrals. CMHWS provides support to people of all ages experiencing psychological distress or mental ill health.


We are seeking a highly-motivated, experienced Lead Practitioner – Family Practitioner for a 10 month fixed term, full time opportunity to work in the Melbourne metro area. Working from home, you will provide practice leadership and direction to staff, coach a team to deliver person-centred outreach services, and support clients to achieve their wellbeing and My Better Life goals. As this position requires travel across the Melbourne metro area a fleet car or allowance is provided.


Key responsibilities

  • Lead and guide the team to deliver high quality services and facilitate positive educational and social experiences.
  • Complete intake assessments to support service planning and develop recovery plans using the My Better Life tool.
  • Support clients with recovery-orientated frameworks, provide information and mentoring to establish connections with families and carers, create networks, and access to relevant services.
  • Work as part of a multidisciplinary team offering specialised psychosocial support.
  • Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to client’s recovery goals.


What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Ability to coach and lead a team to achieve quality service outcomes and provide professional supervision.
  • Expertise in providing outreach to adults and young people with mental health issues, complex needs and interacting with families and carers.
  • Capability to implement therapeutic, recovery oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
