We’re looking for a skilled and experienced person with a background in case management with young people who have been in care.    

The Transition to Adulthood Team at GCYS supports young people aged 15 to 25, who are transitioning from out of home care or who have left care. This role is an exciting opportunity to join the Transition to Adulthood (T2A) team at GCYS which focus on ensuring young people are adequately prepared for their transition from care and supports them after they have left care to achieve positive long-term outcomes in adulthood.

Our team works in partnership with the local Child Safety Service Centres to identify when young people require assistance, and provides direct and intensive support from the basics, like getting identification and income established, to the big stuff – like searching for appropriate and affordable housing, accessing mental health support and re-engaging in training and employment. This is an outreach-based role, working in community and meeting young people where they’re at.

About the job
Role location: Miami and across the Gold Coast
Role type: Permanent full-time 
Pay range: SCHADS level 5 based on experience and with salary sacrificing available (excludes Novated Leasing).

You will:   

  • deliver effective case management services and provide advocacy for young people transitioning from out of home care 
  • work collaboratively alongside key stakeholders from Child Safety, the out of home care sector and community support services to provide a positive transition to adulthood for young people exiting the statutory care system
  • provide one-on-one coaching to assist young people to identify their goals, agree on a way to achieve them, and overcome any barriers.
  • provide young people with practical support around housing, employment, mental health, general health and wellbeing to establish a firm foundation for growth.

About you
You will need to have a minimum of 3 years’ recent experience working with young people, including solid case management skills and an understanding of the challenges and barriers that young people in out of home care face as they transition to adulthood.

You will be proactive in a reactive environment. You will enjoy being part of a team that likes to generate creative and innovative solutions.

You will have the ability to work independently and recognise and anticipate the barriers that young people with a care experience face and be ready to jump into action to provide practical support to help young people get back on track and take a planned and purposeful approach in all the support you provide. 

We are looking for someone who is able to engage with young people from a range of backgrounds including a history of complex trauma. You will be able to build productive working relationships with a network of other professionals and stakeholders – you know that you can’t do it alone and the value the input of others.
