The Opportunity

The Child and Family Services team is part of the Innovation and Community Care directorate. This assertive outreach service works within the social model of health and Best Interest Framework to provide a meaningful service to expectant parents, infants, children, and their families to empower them to reach their goals relating to their family, individual, parenting and wellbeing aspects. 

Through a strength- based approach, you will be critical to guiding this group, who may face adversity, in identifying and managing their needs that may impact their developmental stages of life, capacity to self-manage and enable them to build on their strengths and aspirations.

In this diverse role, you will offer a case management model that incorporates a child and family comprehensive safety and risk assessment; care planning; group work; supportive counselling and linkages with community supports to support children, young people, and families.

To achieve this, you will work alongside and with a variety of complementary services and programs such as: Early Years services, Education, Counselling, Addictions, Allied Health, Dental and Medical to provide an integrated and comprehensive system of support to families.

You will be well supported by a highly engaged team with an experienced Child and Family Services leadership team including 2 Team Leaders and the Manager Child and Family Services, focusing on your individual goals for personal and professional growth and development.

Graduate Opportunity

  • Child and Family Services Outreach Worker (Level 3) – 0.7FTE Part Time Fixed Term until 30 June 2023; Wyndham Vale and Brimbank locations

Experienced Practitioners Opportunities

All positions are flexible to consider a minimum commitment of 0.6FTE.

  • Family Services Outreach Practitioner (Level 5) – 0.8FTE Part Time Ongoing; Wyndham Vale location
  • Child and Family Services Outreach Worker (Level 5) – 0.8FTE Part Time Fixed Term until 30 June 2023 (Funding based activity – Project); Wyndham Vale and Brimbank locations
  • Family Engagement Practitioner (Level 5) – 0.8FTE Part Time Fixed Term until 30 June 2023 (Funding based activity – Project); based at Brimbank Council location

All positions are classified as a Social and Community Services Employee Level 3-5 classification TRP $74,325 – $102,517 pa (Pro Rata for Part Time) under the Community Health Centre (Stand Alone Services) Social And Community Service Employees Multi Enterprise Agreement 2017.

You will make a difference by

  • Providing meaningful case management and engagement services to expectant parents, infants, children and families that responds positively to identified barriers and influence behavioural change.
  • Contributing to ongoing safety and well-being of our clients through effective risk identification, assessment and mandatory obligations.
  • Delivering on the Child and Family action plan in collaboration with stakeholders.
  • Supporting a culture of continuous quality improvement and best practice frameworks to enhance the service model and achieve the best client outcomes for parents and children.

To succeed, you will need

  • A tertiary qualification in Social Work, Psychology or related field with experience in the community service sector.
  • Experience in best practice case management practices including ability to undertake initial and ongoing comprehensive assessments, development of person-centred care plans that contribute to positive outcomes for infants, children, parents, and families.
  • Foundational or sound understanding of mandatory reporting legislation with ability to apply the MARAM Framework and CISS to assess risk and undertake safety planning, and fulfil duty of care.
  • Current and valid Victorian Working with Children’s Check and drivers licence.
