JOB DESCRIPTION

Case Manager/Health promotion   position description 

In relation to Case Management 

Provide support and case management for women 
Assist clients to identify their needs and goals and develop an individual support plan and strategies to achieve goals, including exit planning
foster and work in partnership with other organisations in the development and delivery of services and projects as relevant to the role
outreach to assist clients with service access/advocacy issues
Network with agencies to develop referral pathways
Maintain client files, record information , enter information on WHNSW database

In relation to Health Promotion 
provide outreach women’s health education and health promotion activities in accordance with Centre goals
provide small group facilitation in accordance with the BWHC Annual Plan
preparing submissions for funding relating to the role or other staff roles in consultation with the CEO
participate in local Ministry of Health projects, including liaison with Women’s Health Promotion Unit
contribute to other Strategic Plans that have a direct affect on women’s health issues. 
liaise with other relevant professionals in the development of new partnerships and collaboration on projects.
share information and knowledge through participation in interagency committees
Coordinate the Centre’s program of groups and activities from term to term in consultation with staff/client group identified needs and BWHC strategic plan, in accordance with the Health Promotion Policy and Procedures
liaise, monitor, book, confirm, and collect proposals from educators.
develop and execute client participation surveys and other forms of evaluation
conduct ongoing research both qualitative and quantitative. 
promote the organisation in media and publicity efforts 
provide and maintain social media sites.
Assist with the supervision of students as negotiated with the CEO
 

Qualification and Experience 

Social Work degree or equivalent
Demonstrated experience working as a caseworker
Demonstrated experience working with women who have experienced domestic family violence 
Experience in client management systems  
Commitment to women’s health issues, and feminist philosophy 
Drivers license, and second language an advantage
Part time or full time positions considered 

2 positions available Bankstown and Fairfield women’s health

BWHC services Canterbury and Bankstown locales and auspices Fairfield Women’s Health Service in conjunction with Liverpool Women’s Health Centre.
