Health and Community Services (HCS) aims to assist people with a disability to identify support that really matters most to them. As a registered service provider under the National Disability Insurance Scheme (NDIS) with experience in providing services to Victorians living with disability or developmental delays, including mental health we understand the NDIS and how it may impact our consumers
HCS is seeking Behaviour Support Practitioner with a working knowledge of NDIS legislation and standards. We want to hear from applicants who are empathetic, truly care about and listen to participants. They should have ability evaluate their own work with a desire to improve and look to deliver the best possible outcomes for participants and their families
To succeed in this role, you will have:
- A qualification in Social and/or Health Sciences or equivalent
- Experience in a Positive Behaviour Support Practitioner position
- Deemed suitable by the NDIS Quality and Safeguards Commission to deliver Positive Behaviour Support at the level of Core Practitioner and above
- A minimum of one year experience in disability, mental health and/or early childhood
About the Role
- You will be supporting NDIS participants from the age of 6 to 65yrs.
- Conduct functional behaviour assessments, development and implementation of Positive Behaviour Support Plans.
- Train and support families and other stakeholders in the implementation of behaviour support strategies, aimed at reducing or eliminating behaviours of concern
- Monitor the effectiveness of behaviour support strategies.
- Provide supervision to other PBS practitioners.
- Previous experience with NDIS will mean your efforts will have the best outcomes for our customers.
- Complete reports and work towards goals within the NDIS plans
- Actively participate in team meetings and all related training
