About the Role
Our Monash Health Continuing Care Case Managers work within a multi-disciplinary team to provide treatment and support to individuals experiencing complex mental health problems. We strive to deliver high quality recovery-focused client care. Our aim is to promote self-efficacy and independence, and social inclusion with all the individuals we support. Working in our diverse geographical area, you will enjoy the challenges of working with the complexities of mental health and making a positive difference every day. You will work closely with the range of mental health services to effectively co-ordinate client care, including inpatient units, CATT, CCU and PARCS.
About You
You are a dedicated and passionate Mental Health Clinician who enjoys working in the adult mental health arena. You enjoy working as a member of a multi-disciplinary team and you have a strong commitment to providing high standard holistic care to both patient and their families/carers. You have great skills in assessing needs, developing plans and delivering therapeutic, evidence-based interventions. You are have a resilient personality and you are a person who lives our iCARE values of integrity, compassion, accountability, respect and excellence.
