 Your Opportunity 

We have an amazing opportunity for an experienced Case Worker to join us working with people who have experienced severe and recurring mental illness and/or addiction. 

Working 30.4 hours a week across either four or five days the Case Worker will provide support to two of our Programs:

Enhanced Adult Community Living Supports (CLS) supports people with severe mental illness to live in the community. In partnership with the Local Health District mental health teams, clinical care and psychosocial supports are provided.

The AOD Continuing Coordinated Care (CCC) Service provides care coordination and wraparound services for people with a substance use disorder who are receiving community-based treatment, while also experiencing a high level of need.

You will work with our consumers to reduce overall functional impairment, implement recovery and/or harm minimisation strategies and achieve outlined person-centred goals.   

Your key responsibilities will be to: 

  • Respond to referrals of clients to the service from internal and external support services
  • Provide outreach services to clients – successfully inducting them into the program and creating individualised support plans
  • Support clients to access and maintain Mental Health and/or AOD health care plans and to achieve optimal recovery outcomes
  • Develop and maintain relationships with internal and external stakeholders including service partners and government agencies
  • Administration – Complete a range of internal and external reports relating to clients and the program including risk assessments, individualised support plans and program promotion.

Requirements for Success:

  • Relevant experience or tertiary qualifications in social work, psychology, welfare or other related disciplines.
  • Proven knowledge of complex mental health issues and co-occurring factors including AOD misuse and Substance Use disorder
  • Understanding of the Recovery Orientated Principles and Framework, as well as Psycho-social rehabilitation and harm-minimisation practices to adequately support client recovery
  • Case management skills including the ability to quickly acquire knowledge of locally available services.
  • Skills in building rapport with clients while also liaising, negotiating and advocating for consumers with a wide variety of stakeholders
  • First Aid certificate and Driver License
  • Available to work before/after hours and weekends if required
  • Evidence of Covid 19 Vaccination
  • Willingness to undertake any relevant training opportunities for professional development relevant to role

Before starting work with us, you will need to undertake a national police history check, qualifications and referee checks, and have a clearance to work with children/vulnerable people.
