Lismore Mental Health Services are seeking a highly motivated Peer Support Transfer of Care Worker (STOC) to:
Provide individual and group support to consumers and carers by working collaboratively within a multidisciplinary team to ensure patient centred recovery. Developing supportive empathetic relationships with consumers based on Lived Experience of Mental Health Issues by empowering consumers with skills and strategies to aid their personal recovery.
If you have:
- Applicants for this position must be of Aboriginal descent through parentage, identify as being Aboriginal and be accepted in the community as such
- Personal experience of mental health issues and recovery, understanding issues faced by people with mental health issues and using recovery principles. Ability and willingness to share personal lived experience of mental health issues and recovery in a professional manner Recognised relevant minimum Certificate IV Qualification and work experience relevant to this position
- Demonstrated knowledge of, and ability to plan, implement and evaluate group and individual peer led interventions or consultations
- Demonstrated communication skills and ability to build effective partnerships and work cooperatively with others, including service staff, consumers and mental health networks within a multidisciplinary team environment
- Demonstrated knowledge and understanding of consumer participation and partnership, Person Centred Care, Trauma Informed Care, Consumer led Recovery, rights and responsibilities for consumers, NSW Mental Health Act 2007 and Amendments, National Mental Health Standards (2010)
