About the role
The Sub-Acute short stay Youth Step Up Step Down (YSUSD) service, delivered in partnership with QLD hospitals, supports young people aged of 16-21 years transitioning from a hospital setting back into the community or to prevent admissions whilst residing in the community. Clients can access staff 24/7 and are empowered to enhance connections with families and carers and increase their capacity to live fulfilling lives within the community. YSUSD works within a recovery focused and clinical treatment framework, supporting clients based on strengths and goals identified in recovery plans.
We are seeking empathic, self-motivated Community Mental Health Practitioners for casual opportunities to work alongside caring team on a 24/7 rotating roster in the Woodridge area. You will deliver person-centred, recovery-oriented support to young people to develop their social and living skills, achieve wellbeing goals and live independently in the community.
Key responsibilities
- Support young people to return to their community and enjoy a meaningful life through an individualised approach to recovery.
- Deliver high quality services and facilitate positive educational and social experiences.
- Work as part of a multidisciplinary team offering specialised psychosocial support.
- Plan and deliver group programs that aid clients to build their skills and focus on recovery.
- Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to the client’s recovery goals.
What you’ll bring
- Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
- Able to provide a record of Vaccination Preventable Diseases or able to obtain vaccinations against Measles, Mumps, Rubella, Chicken Pox, Whooping Cough, Hepatitis B or other diseases as required by our Partnership with QLD Health.
- Expertise in working with young people with mental health issues, complex needs and interacting with families and carers.
- Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
