About the role
Mater at Home has an exciting opportunity for a newly established role of System Navigator. This position works within the Homefront Mater Program, a collaboration between Mater at Home, Mater Hospital Brisbane and Micah Projects. The role is being offered on a 12 month fixed term, full time basis, working 38 hours per week.
Salary Range: $34.33 – $51.88 per hour plus super salary + salary packaging access up to $15,900
What you’ll be doing
Homefront Mater is a multidisciplinary team who provide clinical and non-clinical support to people who are experiencing or at risk of homelessness who have significant health and psychosocial concerns. As System Navigator, you will assist participants who present to Mater Hospital Brisbane to better understand services available to them and provide a rapid, person-centred response to initiate supports upon discharge.
Duties and responsibilities include:
- Working with Homefront Mater and other service providers to embed a coordinated referral process to support participants in accessing clinical and primary health care in the community
- Undertaking in reach to meet and complete intake with participants who are in the Mater Hospital Emergency Department or on the wards who have been referred for a rapid response.
- Supporting participants to identify their immediate needs, working with them and other stakeholders to address crisis and make referrals for additional support as required.
- Developing a clear understanding of referral pathways, available supports, service gaps and emergency issues to support system level planning
- Supporting Mater Hospital Brisbane staff to improve their understanding of services and referral pathways within the region to support participants’ social, health, and housing needs
About you
To be considered for this position, you must have:
- Tertiary qualifications in Social Work with eligibility for registration with the Australian Association of Social Work (AASW)
- Valid Queensland Driving Licence
- Blue Card and NDIS Worker Screening
- COVID Vaccine Certificate
Desirable:
- Experience working within health care and/ or human services field and with the target population group.
