An exciting opportunity exists for a motivated and skilled care coordinator, the role exists to expand Star Health`s Work Safe Program. The program is funded by Work Cover to provide mental health specialist coordination and support to injured workers who have a mental health diagnosis and complex care needs.

Ideally, you will possess:
- A professional qualification in relevant tertiary qualification eg Welfare, Social Work.
- Experience in and demonstrated ability in the provision of recovery focused care coordination, advocacy and support to people who have a mental health diagnosis and complex care needs.
- Demonstrated ability to develop strong working relationships with clients and support them to meet their recovery goals.
- Excellent knowledge of recovery model of care and the health factors underpinning barriers to access support and recovery..
- A clear understanding of the Mental Health and NDIS sector. Knowledge of Work Cover an advantage!
