JOB DESCRIPTION

  • Permanent full time, with flexibility for part time hours (minimum 30 hours per week)

360 Health + Community’s experienced NDIS Support Coordination team is all about enhancing opportunities for people living with disability to pursue life goals and achieve greater independence.  In this role, your compassion, collaboration and integrity will shine through as you work with participants to understand and use NDIS plans, and connect with support services that will build capacity for self-directed care into the future.

Our service has a psychosocial disability focus and we are seeking applicants who:

  • Minimum of Certificate IV in Mental Health or demonstrated experience in the mental health/NDIS sector.
  • Degree qualification in a related field which could be in Social Work, Psychology/Counselling, Occupational Health, will be highly regarded.
  • Understand recovery oriented and client centred practice.
  • Are self-motivated and highly organised to maintain a busy participant schedule.
  • Can keep excellent case notes and be efficient with fee for service administration to meet KPIs and reporting requirements.
  • Enjoy being part of a larger team and building relationships with internal stakeholders, therapy partners and community support organisations.

In addition, shortlisted candidates must have or be willing to apply for an NDIS check, a Working with Children Check, National Police Certificate dated within 6 months, and be fully vaccinated against COVID-19.

The hourly rate for this position is $33.38 to $37.16 per hour, depending on skills and experience.  In addition we offer a range of benefits including hybrid work settings, salary packaging to maximise your take home salary, support for further education, professional development days and your very own birthday leave.
