About the role
This position sits within the cohealth Homelessness Cluster, which aims to improve the health, wellbeing, housing, and social outcomes of people who use alcohol and other drugs and/or those experiencing homelessness in the Inner Northwest of Melbourne.
This position is based within the Homeless Outreach Mental Health & Housing Service (HOMHS) team, an interdisciplinary team providing people who experience severe and enduring mental illness and a history of repeated homelessness, a pathway to better health, secure housing, and increased social and economic tenure.
The successful applicant will maintain an individual caseload as well as support the team-based care approach for all HOMHS consumers.
The position is funded until June 30th, 2023, with the hope additional funding will be offered to extend the position and the overall HOMHS program (currently in its eighth year).
Full time position is offered but can be negotiated if a smaller EFT is preferred.
WFH is offered (in a small capacity) and can be discussed once successful in securing the position.

Your duties include:
- Provide strengths-focussed, recovery-oriented, evidence-based, and client-centred case management support and advocacy for those who are accessing HOMHS
- Work collaboratively with consumers, family, external services, and the HOMHS team to conduct comprehensive assessments which identify consumer needs and goals
- Support clients to develop advance statements and wellness plans
- Complete relevant health care administrative processes, including LSP16, HONOS, Risk Assessments, Recovery & Wellness Plans, Client Discharge Summaries, GP, and clinician support letters
Our ideal candidate:
- Tertiary qualifications in a health-related discipline with a minimum of two years professional experience in similar position
- Detailed understanding of evidence-based best practice interventions for consumers with complex needs, including strengths-based frameworks that facilitate consumer choice
- Experienced in providing comprehensive assessment, goal setting, and care planning
- A good working knowledge of mental health conditions, treatment interventions and the interface between Community Mental Health Services and Area Mental Health Services (AMHS)
