About the Program

Melbourne City Mission’s Palliative Care service (MCMPC), was established in 1981 as the first community-based palliative care service in Victoria.  The service aims to enhance quality of life by providing specialist, interdisciplinary palliative care and practical support for people living with a terminal condition.  Support is offered to families and carers, including the management of complex symptoms, loss, grief, and bereavement.
The service is provided to residents of Darebin, Hume, Moreland, and Yarra in accordance with the Palliative Care Australia Standards 2005.   Palliative Care operates within the MCM Disability, Early Childhood Intervention and Palliative Care division and implements MCM’s commitment to best practice in palliative care.

About the role

This is a dual role within the Allied Care Team, working 2 days a week within Aged Care and 2 days a week with the Bereavement Team. Working within Residential Aged Care team, you will increase the capacity of residential aged care facilities to manage quality of life care for residents with specialist palliative care needs. Working within the Bereavement Team, you will provide holistic and therapeutic grief, loss and bereavement support to carers and their families, through counselling, psychoeducational programmes, and group work.

  • Providing appropriate assessment and responsive intervention in the form of counselling and psychosocial support and addressing welfare issues of clients and carers, friends and family, some of whom have high risk and complex needs.  This may also involve appropriate referrals to internal or external stakeholders.
  • Initiate and participate in ongoing advance care planning and assist clients and carers to resolve issues that arise from these discussions where relevant
  • Provide appropriate support (including adjustment to illness and transition to care), information, education and advocacy for clients and carers of diverse ages, cultures, and backgrounds 
  • To regularly facilitate/co-facilitate bereavement groups and bereavement education to our bereaved clients. To also provide education to professionals/networks as needed.

Skills and experience

Essential qualification: 

  • Tertiary qualifications in a discipline, with an emphasis on psychotherapeutic practice such as Social Work, Counselling, or Psychology 
  • Eligibility for membership or registration to relevant professional association

Essential criteria:

  • Demonstrated experience in providing bereavement counselling and psychosocial support in palliative care, aged care, community health or related field
  • Understanding of loss and grief across the lifespan
  • Demonstrated competence in providing counselling support to people of varied social, cultural, and religious backgrounds
  • Experience in conducting psychosocial, spiritual and high risk needs assessments
  • The ability to work collaboratively within an interdisciplinary team setting
  • Demonstrated competence in implementing and maintaining self-care routines including reflective practice 
  • Demonstrated experience in aligning practice with professional standards
