  • 45 to 60 hours per fortnight negotiable, continuing position 
  • Ballarat

About the Position

The Practitioner Family Mental Health Support Service will provide intensive, long-term, early intervention support for children, young people, and their families. Practitioners work with the child/young person and their family to identify risk factors and provide education, resources, and referrals to support their wellbeing.

The program also provides psycho-educational and therapeutic group-based programs for children, community outreach, mental health education, and community development activities, 

Specifically, the position is responsible for:

  • Conduct client assessments and provide case management support
  • Provide active and assertive in-home or community-based support to children, young people and their families to support their engagement and access to services
  • Collaboratively develop and implement with families the Family Action Plan. Provide tailored and appropriate responses to children, young people, and families.
  • Facilitating psycho-educational group-based programs for children, parents, and carers.

To be considered for this position, you will require:

  • Tertiary qualification in Social Work, Community Services, Education, Youth Work, Counselling, or relevant experience with a focus on mental health and working with children and young people in context. 
  • Demonstrated skills in developing and delivering pyscho-educational group programs with strong engaging and confident group facilitation skills
  • Demonstrated ability to quickly build rapport and provide a psychological safe place for sharing with students.
  • Demonstrated ability to deliver case management services to families with mental health issues
