SUMMARY
Primary and Community Care Services is seeking a Social Worker with background in disability and complex mental illness on a maximum term contract with a possibility of extension.
As a Social Worker, you will provide care co-ordination in the delivery of integrated care programs. You will manage the care planning responsibilities of people with mental health conditions.
THE ROLE
- Provide holistic wrap around support to clients, carer’s and the community
- Facilitate multidisciplinary care plan including initial assessment, review and ongoing services & support
- Ensure clinical quality and client-centred care is provided to program participants, particularly those that help minimise preventable hospital admissions.
- Liaise with various health and social service providers to ensure clients receive timely home and community-based care.
- Work closely with GP’s and other referrers to support the client to access the most appropriate service to meet their needs.
- Ensure communication of client information to the wider members of the care team in agree timeframes.
- Undertake care planning activities and ensure services to program participants are managed seamlessly and effectively.
- Accuracy and attention to detail in record keeping and evaluation duties essential.
WHAT TO BRING
- A strong understanding of principles of care coordination, integrated care and holistic care.
- NDIS experience
- More than 1 years of experience in similar roles and will demonstrate a solid understanding of the principles that underpin coordinated care delivery
- Professional registration/eligibility AASW.
- A strong knowledge of disability & mental health services.
- WWCC and Police Check
- Drivers License
