JOB DESCRIPTION

When you join Black Swan Health, you become part of a team dedicated to delivering life-changing care for individuals and our community. Our network of person-centred health, wellness, mental health and disability support services is as diverse as our workforce, spanning across metropolitan Perth and regional WA.

We seek passionate individuals, committed to living their values (and ours!) through the delivery of compassionate, evidence-based primary health services.

If that sounds interesting to you, read on! We might just have the job for you!

The Opportunity

We’re looking for our next Youth Access Coordinator to provide a high quality, early intervention response to young people and their families seeking support. If you’re a people person who is passionate about mental health, we should talk! The Youth Access Coordinator position sits within our headspace team, based out of our Joondalup office. headspace is an initiative funded by the Australian Government designed to increase access to, and use of, health and other services for young people aged from 12 to 25 years. Using an early intervention approach, headspace tailors a “wrap-around” service model to meet the needs of young people at risk of developing mental illness, substance use disorders or other issues. The headspace model delivers direct services across four core platforms of mental health, primary health, drug and alcohol services, and vocational and education services.

The Role

The Youth Access Coordinator provides assessment, psychological intervention and care coordination to young people and their families including the facilitation of referral to other community services as appropriate.  The role will require you to monitor high-risk clients and ensure prompt appropriate assessment, management and follow up support is provided. You will provide information, support and psycho-educational content to young people and families accessing the service via phone and walk-ins. It will also involve the participation in family consultations to maintain supportive relationships between young people and their family, where appropriate, and with the young persons consent.

About You

The successful candidate will possess tertiary qualifications in a relevant discipline and registration with an appropriate professional body such as AHPRA, AASW or PACFA. You will demonstrate at least 1 years’ experience working in a clinical environment with young people living with mild to moderate mental health issues including completion of mental health assessments, cognitive behavioural interventions, interpersonal psychotherapy and family-based interventions. This opportunity will enable you to demonstrate your capacity to develop rapport and effective professional relationships with young people and associated professional organisations. You are an excellent communicator and committed to person-centred care.

Please be advised that an offer of employment will be subject to verification of your right to work in Australia and the provision of applicable qualifications, registrations, licences, National Police Clearance, Working with Children Check, and COVID-19 vaccinations evidence.
