We are seeking an experienced Case Manager to join the Community Mental Health Team to work in partnership with the Emergency Department and Inpatient at the hospital to ensure all clients are provided with evidence based therapeutic intervention and ongoing support who are at risk of serious mental illness.

Key Responsibilities
- Provide Support to a medium to large caseload of clients providing support for patients, their families and carer(s).
- You will also identify and engage with other key community service providers involved in the provision of rehabilitation, recovery, support, and discharge planning consistent with the patients and family/carer(s) needs.
- The role encompasses community consultation, awareness and education activities to increase the understanding of psychiatric disorders and delivery models within the community.
About You
- Completed Qualification in Social Work or Psychology
- Previous Experience as Mental Health Clinician or Mental Health Case Manager in a hospital setting.
- Current Registration with professional body or eligibility to do so.
- National Police Check
- Current Drivers Licence
- Covid 19 vaccination including booster
Remuneration Package
Casual Rates up to $55 per hour depending on speciality + super.
Free Shared Accommodation is provided and travel allowance both ways to contract.
