Your Opportunity
The Djindigal (meaning Healing Place) service is a 10-bedroom facility in between Mareeba and Kuranda. The service aims to provide a way for people who have self-identified substance use concerns, to take control of their use and reduce the negative impacts on their life. The service will empower and equip them to achieve their individual goals relating to their substance use, and to be able to continue their recovery journey for a further 6 months after leaving the residential setting. This may look different for different people including goals relating to safer use/harm minimisation, reducing use or ceasing use.
We currently have a vacancy for an experienced Case Worker to deliver individualised intensive case management support to clients who participate in the Djindigal Residential Rehabilitation Service.
Your key responsibilities will be to:
- Provide person-centred case management based upon client goals, needs and preferences.
- Develop strong professional and supportive relationships, and make appropriate referrals when required.
- Complete case plans and reporting accurately and in a timely manner.
- Assist clients in the process of transition out of the service into independence or other services including the support team.
- Effectively report all incidents and risks, in line with MA policies and procedures.
