Clinical Mental Health Care Co-Ordinator working in a General Practice. The role includes the clinical care co-ordination working within scope of practice to ensure you provide evidenced based intervention to people with severe and complex mental illness who are currently managed in Primary care. As a co-ordinator you will work with the General Practitioner, psychologists/psychiatrist to provide coordinated clinical care. The role will be to complement and enhance existing GP, psychiatrists/psychologists and allied mental health services available through the MBS and offering the right frequency of services to meet the needs of the patient.The role will be to promote recovery and provide links to other services within the stepped care approach to ensure people are matched to a service commensurated with their mental health needs.
