About the role:
The Way Back Support Service is a non-clinical support service focused on providing practical psychosocial support to people experiencing a suicidal crisis or who have attempted suicide.
Support is provided for up to three months and targets those at the highest risk through referrals following hospital presentations or through specialist mental health services.
The Practitioner – Health & Wellbeing (Way Back Program) is responsible for providing non-clinical care and practical psychosocial support to individuals in the first three months following a suicide attempt or suicidal crisis.
You’ll be:
- Working collaboratively with clients with a recovery or strengths-based approach to formulate, implement and review support plans, needs assessment and safety plans;
- Working collaboratively with clients to monitor progress through completion and review of outcome measures;
- Participating in intake and assessment processes with referral partners;
- Implementing practices to maximise engagement of people referred to the service, especially from harder to engage groups;
- Ensuring interactions with diverse clients are culturally appropriate;
- Assisting clients to connect with health, clinical and community-based support services as outlined in the hospital discharge plan and wellbeing plans;

What we are looking for in an applicant:
- An understanding of the processes people utilise to overcome adversity/distress arising from life challenges.
- Demonstrated case management skills and ability to work effectively with clients, family members and service providers to identify solutions.
- Sound understanding of the personal, family, and social issues associated with suicidal ideation and attempts, including awareness of groups identified to be at increased risk of suicide.
- Experience liaising with external service providers including specialist mental health services, GPs and other community-based services and organisations.
- Excellent communication skills, particularly with individuals and their families/carers who may be experiencing significant distress.
- A diploma or degree qualification applicable to the delivery of non-clinical psychosocial support services.

Successful applicants must be willing to:
- provide a current Tasmanian Working with Vulnerable People Registration (employment/volunteer category) including NDIS Worker Screening Check if you are applying for the 1st time or have renewed your registration since 1st February 2021;
- present an unsupervised contact with children or vulnerable people type National Police Check (no older than 12 months);
- provide an International Police Check if you have resided for 12 months or more outside of Australia after the age of 16;
- undertake a satisfactory Anglicare assessed medical:
- provide evidence of your COVID-19 Digital Certificates (including booster) or a vaccination exemption;
- provide a current provisional or full Australian Driver License.
