About the Role

As the Program Facilitator (End-of Life and Palliative Care) you’ll be part of the Primary Care Development and Delivery Team.   This talented and skilled team drives the Primary Care transformation and integration across Western Sydney with the objective of increasing the equity of health access.

From assisting in the development of organisational strategies that support the implementation of key objectives within the greater Choice at Home for Palliative Care portfolio, to consulting with relevant stakeholders to develop strategies and  communication processes across service providers, including how to access palliative care support and advice after-hours, this  role involves working with Primary Care providers to apply evidence base practice in palliative care and use relevant MBS items and other programs provided by WentWest.

We’re looking for someone who

  • Is able to work at some out of hours events or community forums
  • Has 3 years experience in a service delivery, program coordination or support role in a health care or NGP environment
  • Demonstrates an understanding of the Australian health care system, including palliative and end of life care and relevant services
  • Works collaboratively consults, negotiates, and manages complex relationships

To be the successful candidate you must meet the position requirements as well as provide a current National Police Check. In accordance with the WentWest mandatory COVID-19 vaccination policy you will be required to be double vaccinated against COVID-19. Current NSW Drivers Licence and access to transport as regular travel is required as part of the role within the Western Sydney area is required. Full working rights to work in Australia are required for this role.
