About us

EPIC Support Services assists individuals living with significant and complex Mental Health conditions to lead meaningful lives. EPIC specialises in NDIS Complex Mental Health Support Services and are dedicated to working with vulnerable people and making sure that they don’t slip through the cracks.

We currently have a Part-Time 3 days per week vacancy for a Psychosocial Coordinator of Supports to join our Melbourne NDIS Support Coordination team supporting complex participants to achieve their NDIS plan goals. This position has the ability to increase to full-time.

About you and the role:

Are you a passionate and dedicated person qualified and experienced in Mental Health or Peer Work? Do you have lived and learned experience of mental ill-health and/or psychosocial disability?

With the above specialist knowledge, the Psychosocial Coordinator of Supports will be able to support participants with their journey towards leading a meaningful and fulfilling life. You will have exceptional communication skills and the ability to effectively organise your time to allow you to work flexibly and directly with participants and support them to make choices for their NDIS and community supports.

What you’ll be doing

As the Psychosocial Coordinator, you will supporting a caseload of NDIS customers to build capacity, strengths and resilience through strong respectful relationships, plus ongoing skilled support to assist people with psychosocial disabilities to live an independent, contributing and meaningful life in their chosen community. By working collaboratively with participants, families, carers and the broader service system to identify, plan, design and coordinate NDIS supports, enabling them to take control and overcome barriers to better manage complex challenges of day-to-day living.

About You

  • Demonstrated experience working with people with mental health conditions
  • Tertiary qualified in Mental Health, Peer Work, Psychology, Counselling, Social Work or relevant to the field of human services or allied health
  • Demonstrated knowledge of NDIS plans
  • Candidates with AOD and Justice experience and or crisis intervention work will be highly regarded

Tasks & responsibilities

  • Provide intensive person centred mental health support for participants
  • Visit participants in their homes
  • Support Participants to Implement their NDIS plan and work towards their goals
  • Linking participants to support providers in accordance with their NDIS plan goals and funding.
  • Complete plan goals progress reports
  • Networking and collaborating with community and government agencies
  • Ability to liaise with care teams, informal and formal supports and legal services
  • Well-developed interpersonal and communication skills, both written and verbal
  • Strong organisational and administrative skills
