The Opportunity
The Dual Diagnosis/Integrated Care Clinician will be a part of broader Mental Health Team at Ballarat Community Health (BCH), with strong and close links to the BCH Alcohol and Other Drug (AOD) Team. The position will be embedded in a high functioning multi-disciplinary team.
The aim of the Program is to address the Mental Health / and or AOD needs of community members referred to the service through the PHN intake and assessment services. The outcomes include improved mental health, symptom stability, harm minimization, and or addiction management. We aim to address client or carer needs and assist them to navigate a challenging system. Ensuring their needs are addressed at the right time for them and their own personal journey.
This program also will be provided within and after hours, via group programs, and individual sessions. BCH are endeavouring to tailor an AOD and MH program that can address both client and carer needs.
Responsibilities of the role will include:
- Work with complex clients who may have co-occurring disorders including AOD, Mental Health and Acquired Brain Injury
- Providing services out of usual business hours including evenings, and some weekend hours, as required
- Provide high quality engagement, assessment, treatment and support services (pre and post care) to individuals and their families, using best practice principles including family inclusive practice and dual diagnosis principles
- Conduct comprehensive assessment (including risk assessment) of clients using appropriate Mental Health and AOD assessment tools
- Identifying risk assessments, including appropriate risk management practices, including team liaison. Displaying expert knowledge of when these risks exceed team guidelines
- Develop individual recovery plans with clients and assist in the identification of collaborative treatment goals across a range of social and emotional wellbeing domains
What you will bring to the role:
- Qualifications in Occupational Therapy, Social Worker, Psychology, Counselling, Allied Health Professional and/or Nursing with additional qualifications in Mental Health and/or AOD preferred
- Knowledge of and skills in delivering best practice, evidence-based treatment interventions, including the ability to deliver effective brief interventions for complex clients including Motivational Interviewing, Cognitive Behaviour Therapy (CBT) and Acceptance and Commitment Therapy (ACT)
- Demonstrated experience in assessment, formulation and treatment planning skills
- Collaborative practice, ensuring understanding boundaries of your role, and when to seek other disciplines expert advice
