About the Role
You will be responsible for providing holistic case management to residents of the service to transition successfully to stable long term living in the community, while ensuring they maintain effective supports and community connection.
This role is responsible for:
Providing in-reach support and working collaboratively with the discharge team to transition eligible clients to the service. You will be responsible to ensure all relevant information and documentation is collected and documented to ensure a safe and seamless transition. Building rapport with clients and internal/external stakeholders to ensure the service runs smoothly. Delivery of group sessions and also providing 1:1 group based support to develop independent living skills.
About You
You will have demonstrated experience in the following areas:
- Professional experience in case management of people who live with a mental illness, intellectual disability, or acquired brain injury.
- Experience supporting people access community, public, and private housing.
- Professional experience supporting people through Recovery Oriented Practice.
- Professional experience in supporting people through a Trauma Informed perspective.
- Available on call as per required
Qualifications
- Bachelor Degree in Social Work, Psychology, or equivalent.
- Maintenance of a full, unrestricted South Australian Driver’s License
- National Police Certificate
- DHS Vulnerable person related employment check or:
- DHS Aged Care employment check or
- DHS Disability employment check or
- DHS Working with Children Check
- Maintenance of First Aid and CPR certificate
Vaccination Requirements
COVID-19 vaccination is an essential requirement under the Emergency Management (Healthcare Setting Workers Vaccination) (COVID-19) Directions 2021 to work in a SA Health/health care setting.
