The Role
We are currently seeking a values-driven and proactive AOD Practitioner to provide counselling, liaison, support, case management and a range of interventions to assist in improving the health & wellbeing for individuals, family members and significant others experiencing alcohol and other drug related harms across the Mallee region.
You Will Successfully
- Evidence based interventions are delivered in line with client centred treatment that aligns with AOD training and qualifications according to complexity of the client and family needs within a recovery framework.
- Case management support is provided to clients in accordance with TSA case management models and standards of practice.
- Individual treatment plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients.
- Individual treatment plans are implemented with active client participation.
- Clients are assisted to access support services and establish community supports, and where needed are assisted through advocacy with service providers.
- Progress and outcomes of individual treatment plans are regularly monitored and reviewed and where needed action is taken to ensure the plan continues to meet client needs or goals.
- Strong and effective relationships are built with clients and their families, to assist client through treatment using family inclusive interventions.
- Clients are assisted to build networks and post treatment support plans that include personal networks, after care, self-help groups and community activities.
- Ongoing assessment is made of the client’s issues, needs and circumstances from initial intake to exit.
This is a full time position, 38hrs per week fixed term located in Swan Hill
Salary and conditions of employment are in accordance with SCHADS (Social, Community, Home Care and Disability Services Industry Award 2010) SACS VIC Lvl 5 – Welfare Wkr CII & III.
You Will Have
- Relevant tertiary qualification in Social Work or related discipline.
- Minimum qualification Diploma Community Services.
- Relevant experience in a social service environment.
- AOD Skillset or willingness to undertake within the first 12 months of commencing employment.
- A national police record check is required.
- A current Victoria Drivers licence.
- Working with Children’s check.
The Salvation Army is subject to public health orders. You may be required to provide evidence of vaccine status dependent on requirements.
