IPA have a long-standing history of providing staff to Federal Government Agencies and are currently partnering with our client to recruit Mental Health Case Managers in Brisbane.
In this role, you will provide an access point for advice, assessment and responses to urgent, high risk or escalated clients for short term case management.
Some of your duties and responsibilities will include:
- Conduct assessment of client referrals and conduct urgent risk assessments where necessary
- Manage a caseload of clients and provide specialised case management
- Record appropriate case notes and keep accurate case management records
- Provide advice and support to non clinical staff in the team who are undertaking assessment of client referrals
- Provide short term case management intervention where required and link clients with services for ongoing support
Mandatory requirements for this position:
- Bachelor’s degree or higher in Nursing, Psychology, Occupational Therapy or Social Work
- Minimum 2 years’ experience working as a case manager or in a clinical role
- Extensive experience working within a mental health or community case management setting, and with clients with complex mental health conditions
- Experience coordinating a caseload of complex and potentially at risk clients
- Experience in triage, assessment and case management via telephone
The successful applicants for this position much be an Australian Citizen and have a current Police Check, or the ability to pass one.
