inCommunity Inc. is a not-for-profit organisation based in Ipswich, providing housing and homelessness services to young people, adults and families. Our WHY is to inspire people so that they believe that they can. This position will be working across in our Connect team.
ABOUT THE ROLE:
- The Case Manager is responsible for supporting the provision of a high standard of client led case management with vulnerable, at risk people to facilitate the development of positive relationships, positive social and community engagement and support informed decision making.
- The Case Manager will work with the clients to identify their needs and develop a plan to deliver targeted strengths based supports that focus on the individuals housing needs and aspirations.
- The position requires an individual who can demonstrate in-depth knowledge of the issues impacting vulnerable people who are homeless or at risk of homelessness.
- The Case Manager will develop and maintain positive relationships with stakeholders, including Ipswich Housing Service Centre, accommodation providers and other services to support people and their families to achieve their desired outcomes.
- There is an expectation that staff will be reliable and punctual, both within the office and in responding to clients.
- Ensure case plans are culturally appropriate and were appropriate include input from relevant cultural supports including the Recognised Entity.
- Conduct assessments and intakes, including using of QLD Homelessness Information Platform (QHIP). Use other relevant data recording programs and resources as required to capture, store and report on client outcomes and progress.
- Casual positions available in our youth crisis accommodation program, working shift work (including overnights and weekends), providing support to young people.
ABOUT YOU:
To be successful in this role, you will need:
- Relevant tertiary qualifications/training (Cert IV/Diploma or above in Community Services, Social Work, Human Services) and/or previous experience working with vulnerable people
- Demonstrable experience engaging with Aboriginal, Torres Strait Islander, Maori, Pacific Islander and/or Culturally and Linguistically Diverse people and their families
- High standards of professional behaviour including sound understanding of work health and safety, child protection, privacy etc.
- Good negotiation and active listening skills, including written and verbal communication
- Demonstrated computer competence, particularly electronic client/case management and reporting
- Ability to work independently and as part of a team
- Current Working With Children Check (Blue Card)
- Current First Aid & CPR certificates
- Current Drivers Licence
