The role you could play
We are seeking proactive Case Managers, to work within our homelessness stream to work toward our vision of ending homelessness. You will be responsible for working collaboratively with individuals experiencing, or at risk of homelessness to link people with appropriate ongoing support and manage a crisis when it occurs.
Your key responsibilities
- Provide brief intervention, and case management support, dependent on need, inclusive of assessment, referrals, regular communication and meetings
- Work closely with clients, Toward Home Alliance team members, local general practices and other health/community services
- Undertake all aspects of care coordination including developing individualised care plans with clients and supporting clients to access a range of services
- Implement a flexible, proactive, trauma-informed, and client-centred approach to support the client and ensure rights are upheld
- Maintain accurate up-to-date client information and data using TSA information systems
This is a permanent part time position for 30.4 hours. Salary and conditions are in accordance with the SCHADS Level 5 Award.
The ideal candidate will have
- Tertiary qualifications in social sciences, social work or related
- Minimum two (2) years case management experience
- Minimum one (1) year experience in supervising and coaching teams
- Understanding of trauma-informed care and what that looks like in practice
- An understanding of the issues faced by people experiencing homelessness
- Experience working with community members in a crisis support setting
- A valid Employee Working with Children check
- A current Driver’s Licence
- A willingness to undergo a National Police check
