JOB DESCRIPTION

About the role:
The overall purpose of the Practitioner – Out of Home Care is to provide case management to children or young people within residential care services or programs. 

The role will assist to develop participation in positive age-appropriate activities; general physical and emotional wellbeing; engagement in schooling, employment or other training; relevant life skills; and positive connection with family and other significant people.

We are recruiting for a full-time permanent experienced Case Manager/Practitioner to be based at our Underwood service managing caseload from our Ipswich Residential Services.

About You:

To be successful in this role, you will have the following experience and capabilities within a similar role:

  • Applied knowledge and understanding of youth justice and child protection frameworks including the Youth Justice Act and Child Protection Act (1999) and other relevant legislation
  • Understanding of child and adolescent development including impacts of trauma, attachment disorder and grief & loss
  • Assessment of complex to extreme needs of young people and the implementation of strength based care plans that target areas for development
  • Ability to respond appropriately in crisis situations; highly developed problem solving, decision making and negotiation skills
  • Able to work effectively in a team and independently; collaboration with stakeholders and meeting service delivery demands
  • Strong verbal and written communication skills; report writing and meeting planning

  Additionally you must possess:

  • Current Blue Card
  • Current Licenced Care Service (LCS)
  • Current Queensland Driver Licence
  • Degree in Health, Human Services, Behavioural Science, Social Work, Psychology (this is an essential requirement of the role)
  • Ability and willingness to travel within the service region
  • Ability and willingness to work outside of standard business hours and provide on-call support as required
