The Lead, Mental Health Referral and Access will use their team leadership experience and clinical skills to lead the delivery of the NWMPHN mental health referral and access service. The successful applicant will play a key role in strategic thinking about the PHN mental health stepped-care model and will help to improve system literacy and awareness of PHN-commissioned mental health services.

We are North Western Melbourne Primary Health Network (NWMPHN), Victoria’s largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in our community.
What are PHNs?
PHNs have been established with two key objectives:
- increasing the efficiency and effectiveness of services for community members, particularly those at risk of poor health outcomes
- improving coordination of care to ensure people receive the right care in the right place, at the right time
A typical week may include:
- Providing guidance to referrers and commissioned service providers, as needed, to support the stepped care service model at NWMPHN
- Managing performance and operations, identify risks, and collaborating on solutions
- Developing strong and collaborative relationships with providers referring in and being referred to
Meeting with other PHN referral and access teams to share and promote best practice
- Monitoring patient experience and service performance data to inform continuous improvement
- Contributing to projects that shape the future of referral and access practices in the region
We are looking for someone who has:
- Tertiary qualification and Registration as a Mental Health Professional (Social Work (Mental Health Accredited), Psychology, Nursing, Occupational Therapy)
- Mental Health accreditation with the relevant professional body (e.g. ACMHN, AASW) and AHPRA registration (where applicable)
- Five years’ experience in a relevant mental health setting and evidence of supporting multidisciplinary teams
- The ability to successfully engage with and assess people from diverse backgrounds, including those from high-risk groups, those presenting with challenging and complex mental health disorders including Aboriginal and Torres Strait Islander, homeless or LGBTIQ communities
- Expert competency in risk assessments, crisis intervention and an understanding of the complex systems of private and public mental health systems
- Previous experience working in mental health intake and assessment is desirable.
