With the reinvention of CareCo Services we are looking for a Social Worker to join our dynamic team to help with our complex case load.
The Social Worker will have the responsibility for ensuring that the day to day delivery of services is aligned to the business strategy and also meets the expectations and operational requirements of NDIS participants, employees, participants, their families and other stakeholders.
This is a full-time positions (38 Hours per week)
Tasks and Duties
- Assisting clients to access relevant support services and navigate bureaucratic processes
- Developing care plans and strategies in consultation with clients
- Advocating on behalf of clients to government and charitable institutions
- Communicating consistently and reliably with clients to address their needs and provide counselling, information, emotional and moral support
- Ensuring compliance with regulatory guidelines and clinical treatment plans
- Liaising with a client’s family, community and employer as required
About You
- Strong focus on Safeguarding
- Demonstrated experience in the NDIS space, with a strong working knowledge of the relevant legislation and guidelines (including NDIS Pricing Arrangements and operational guides).
- Previous experience as a Support Coordinator / Local Area Coordinator / Planner is essential.
- Diploma in Community Service or social degree is highly regarded.
- Excellent communication and interpersonal skills to effectively collaborate with clients, families.
- Strong track record in project management and/or establishing new services.
- Strong track record in community engagement and relationship building.
- A passion for helping people build capacity, live a good life and achieve their goals.
- Ability to work autonomously.
- Reliable, adaptable., motivated and committed to providing quality care.
