About the role
Working with MIND Australia, Palmerston has been engaged to deliver the AOD case management services as part of the ART team in the South Metro area. This wraparound care model aims to bridge the gap between clinical mental health services and community-based organisations who provide care outside of the hospital setting. The model recognises the benefits of stabilising and supporting individuals to remain well in the community, and incorporates alcohol and other drug, recovery, and peer support components.
The multidisciplinary ART team (clinical & non-clinical) are co-located with existing service teams at the PeeL Health Campus working across both Peel and Rockingham health services.
As the AOD Case Worker you will be experienced in AOD related counselling, education, and case management. Working as part of a small team you must be self-motivated and able to plan and prioritise a varied case load. You should have demonstrated success working in partnership with multidisciplinary clinical services along with well-developed case reporting and record keeping skills. You will have experience in assessment, direct therapeutic and/or clinical intervention tools, transition planning and referrals.

This position is offered as an initial fixed term contract through until June 2024. The position is for 3 days per week, flexibility of work hours can be considered. Pay Level is equivalent to Level 4 – Social, Community, Home Care and Disability Services Industry Award 2010.
