We are looking for a Family Liaison & Intake Officer to build and maintain strong relationships with participant’s families in the Hunter & Port Stephens region. 
 

Job Description

The Family Liaison & Intake Officer is a leader’s role responsible for implementing the intake processes and coordination of placement matching for NDIS participants entering Prospect Hill’s Supported Independent Living program. The role is a key part of Prospect Hill’s consensus-driven ”brains trust” that works to solve problems on a daily basis. You absolutely must be a team player and fluent communicator of critical information to your team and colleagues.

Responsibilities

  • Develop and maintain strong professional relationships with governing bodies and guardians (Office of the Public Guardian, Official Community Visitor program, NDIS stakeholders, participant parents / guardians, Co-ordinators of Support)
  • Liaise with internal & external stakeholders to build strong relationships to promote future placement referrals and assist in identifying suitable placements
  • Attending meetings, functions and agency offices to promote Prospect Hill’s services
  • Liaising with families and external providers/agencies to guide them through the Prospect Hill intake journey. Following up their enquiries, keeping them up to date with all developments, providing education about all aspects of Prospect Hill’s intake process
  • Managing participant intake including Information gathering, collation and analysis of SIL, CP and Respite applications. Ensuring that all documentation is received and understood by the Participants and their supporters and also by Prospect Hill
  • Post-intake review of Participant wellbeing including pastoral follow-up
  • Seeking Feedback on a regular basis from Participants, families and other support organisations
  • Preparing and sending out regular (2 monthly) Participant-centred Newsletters to families including activities, goals reached, photos
  • Being the conduit of information exchange between external supporters of participants and Prospect Hill

Hours of Work
Monday – Friday 9am-5pm. Some out of hours work may be required from time to time where deadlines need to be met.

Qualifications

  • Relevant tertiary qualifications preferred but not required
  • Significant knowledge of the NDIS and its processes regarding intake and support of participants
  • Significant knowledge of the NDIS Practice Standards and Quality Indicators
  • High level organisation and time management skills, which includes the ability to prioritise workload to meet changing demands
  • Strong written and verbal communication skills and the ability to build relationships quickly
  • Current Drivers Licence
  • Current NDIS Worker Screening Check

Salary
Salary will be commensurate with experience and qualifications and will be in the range of $65-$70k.
