JOB DESCRIPTION

About the role

  • Provide a range of clinical and non-clinical psychosocial supports to assist individuals to support their mental health and achieve their recovery goals
  • Provide outreach support services which requires a willingness to work with participants in their homes and the community
  • Complete intake and assessment duties to ensure that the support response focuses on participant’s strengths, goals and objectives
  • Assist with the development and implementation of shared support plans across the multi-disciplinary team
  • Use local knowledge and sector expertise to increase opportunities for people with severe mental health to be connected to the most appropriate supports
  • Co-facilitate therapeutic groups including DBT skills
  • Regularly monitor that the delivery of services and support are within the highest level of customer service using a trauma-informed and recovery orientated approach

Role criteria

  • Tertiary qualification in social Work
  • Demonstrated knowledge and experience in supporting participants through the key responsibilities outlined in the position description – in particular, developing Recovery Plans to support participants in identifying and achieving their goals and undertaking comprehensive psychosocial assessments
  • Comprehensive understanding of supporting individuals who have experienced complex trauma
  • Well-developed practice framework that is grounded in trauma-informed, strengths based practice and the ability to articulate the key theories and models that underpin your practice
  • Understanding of group work and confidence to support the facilitation of psychological therapy and psychosocial support groups
  • Demonstrated knowledge and experience in computer skills, particularly data management systems, word processing skills and Microsoft Outlook management
  • Demonstrated ability to contribute as part of a team to provide a safe, equitable and rewarding workplace

Additional requirements

  • A current open QLD driver’s licence and road worthy vehicle, insured to a minimum level covering third party property damage (comprehensive is recommended)
  • Working with Children card and Working with People with a Disability check or the ability to obtain one is mandatory

 About the recovery and Discovery Centre

Communify’s Recovery and Discovery Centre is a Mental Health Hub that is designed to provide integrated, multi-disciplinary supports for people who live with severe and complex mental health concerns that are having a significant impact on daily life and wellbeing. The Recovery and Discovery Centre is funded through the Brisbane North PHN and delivers support across three key funding streams: Individual and Group Psychosocial Support (including peer support), Clinical Care Navigation, and Individual and Group Psychological Therapies. With a framework based on the Recovery Model, the support that is provided is strengths-based, client-centred and trauma-informed.
