About the Service

The Family Support Network (FSN) is a service delivery model funded by the Department of Communities and is offered throughout the Perth Metropolitan area. The service prioritises Aboriginal families and children, and Centrecare delivers this service in partnership with Wungening Aboriginal Corporation to provide an integrated approach.

The Family Support Network is designed to assist families with:

  • improving parenting skills to safely care for their children at home,
  • receive an integrated and coordinated service response to divert them from the child protection system,
  • and develop and strengthen culturally safe and support networks.

There are two streams to the service model being Intensive Case Management and Assessment and Coordination. This role is a part of the Intensive Case Management stream, which only accepts referrals from the Department of Communities, and is for families who require intensive support to keep their children safely at home. The service provides active and persistent case management to achieve the case plan goals and the support provided is within the family’s home. .

About the role

To provide assessment, case coordination and case planning for high-risk families and families demonstrating a high level of complexity whose children are most at risk of child protection intervention and/or families who have been identified for reunification. Priority access will be given to Aboriginal families.

A Few Specifics

  • To undertake through assessments and case planning of the children, adults, families and appropriate support networks to address the issues impacting on their children’s safety.
  • To deliver therapeutic services to families and individuals to include assessments, counselling, outreach, capacity building, information and referral opportunities for families, utilising a culturally appropriate, persistent case management model.
  • Provide broad, holistic management of complex and multifaceted issues pertaining to the family.
  • Provide culturally appropriate services that meet the needs of Aboriginal and Torres Strait Islander people and Culturally and Linguistically Diverse clients.
  • To establish relevant networks and links with the community and other service providers to ensure the best possible outcomes for clients.

Essential Criteria

  • Recognised tertiary (Degree) level qualifications in Psychology, Social Work, Social Sciences, Counselling and/or related disciplines.
  • Demonstrated experience working collaboratively with other organisations to achieve positive community outcomes.
  • Demonstrated experience conducting extensive screening, assessment and case planning for children, young people and families, especially Aboriginal families.
  • Demonstrated experience providing strength-based, culturally secure services to children, young people and families.
  • Ability to work Monday to Friday with the ability to provide after hours, on call support.
