Program Description:
The Young Aboriginal Support Program provides a personalised and holistic service to young Aboriginal people aged 12-25 in Northern Tasmania.
We are seeking a youth focused Case Manager who is committed, flexible, creative and passionate about supporting young people, to come and join our team. This is an exciting opportunity to work as part of a multidisciplinary team, specialising in youth mental health.
Under the direction of the Clinical Lead and Youth in Mind Outreach Services Manager you will be responsible for:
Providing evidence-based case management support for young Aboriginal people accessing the program
Delivering evidence based psychosocial interventions, that are culturally appropriate for both groups and individuals
Developing appropriate referral pathways for young people in areas of physical and mental health, education, training or employment and/or community engagement
Supporting delivery of activities and events in the community, in partnership with relevant stakeholders

Cornerstone Youth Services Inc. (CSYS) is the lead agency for Youth in Mind Outreach Services, based in Devonport, Burnie and Launceston. CSYS have been providing services to young people aged 12-25 in the community for over ten years. Website: www.cornerstoneyouthservices.com.au

What you’ll need
- Tertiary qualification in a relevant area, e.g. Community Services, Social work
- Experience in working with young people in a case management and/or counselling capacity
- Skills in managing risk and providing individual, family and group interventions for young people presenting with a broad range of issues.
- Ability to engage with young people and their families holistically
- Ability to engage with relevant stakeholders to advocate and support access for young people
Desirable
Previous experience working in the youth sector
Previous experience in developing and delivering group programs
Experience in working with and an awareness of the local Aboriginal community
