What you’ll be doing
The Assertive Community Treatment Team provides evidence based intensive services, as part of a multidisciplinary team, to people with a severe and persistent mental illness with other complexities, such as serious disability, social exclusion and other comorbidity issues (substance misuse, physical health issues etc.). Additionally, because of their illness, consumers may have had frequent and unplanned admissions and/or intensive interventions which are not crisis related and they now require high intensity and frequent follow-up to remain well in the community. There may also be a history of non-engagement and/or non-adherence with traditional mental health services or treatments. The mental health clinician will provide services within a whole team approach in a way that is least intrusive, collaborative, recovery oriented, nominated primary carer and family responsive and inclusive, rehabilitative, encourages self-advocacy, facilitates empowerment and capacity for optimal independent function.
People with disabilities who meet the selection criteria are encouraged to apply; and where required, WSLHD will implement reasonable adjustment consistent with industry standard.
