Psycho-social Support Worker

  • Full-time contract until 30th Jun 2024

 

The Opportunity: 

Working in coordination with Rural Young Minds complex/severe mental health services for young people 12-25 years at Samaritans, the primary focus is to engage with young people in the assessment, goal setting and coordination of care towards recovery goal/s. The Psycho-social Support Worker will also work alongside young people, families and/or carers linking them with services within health and community settings and providing small group workshops.

About you:

  • Qualifications and experience in community services, youth work, health education, allied health or other related discipline
  • Understanding of the local area and local services with an ability to partner with internal and external services
  • Understanding of SMART goals and common social and health care terminology
  • Understanding of the importance of cultural factors when working with people
  • Understand the impacts of Mental Health for an individual and when to refer for immediate help
  • Knowledge of common long-term health conditions and the importance of managing health and wellbeing
  • Ability to form supportive and professional relationships
  • Demonstrated experience prioritising and managing workloads
  • Ability to work collaboratively

The successful applicant will possess or willingness to obtain the following: NSW Working with Children Check, NDIS Worker Screening Check, First aid & CPR certificate, Covid-19 Vaccination Certificate, and undertake a pre-employment functional medical.
