The AOD Case Manager will work as a key contributing member of the Integrated Homelessness Supported Accommodation Program (IHSAP) case management team, providing culturally appropriate AOD support and services to people residing at YISSA who are impacted by their own or someone else’s substance use.

Your Key Performance Indicators will be: 

  • Implement and monitor high quality culturally appropriate, evidence-based, person-centered plans that support personal life choices.
  • Establish strong therapeutic relationships with clients, commencing from induction through to finalisation of exit processes. 
  • Ensure all services are provided in a timely manner.
  • Ensure effective communication channels with other staff, supervisor, management, participants, carers and other relevant stakeholders.
  • Maintain appropriate records in accordance with established documentation protocols.
  • Participate in staff meetings, and in regular supervision and training. 

Some of your responsibilities will be: 

  • Accept referrals through the Integrated Homelessness Supported Accommodation Program (IHSAP)
  • Provide brief interventions, assessments, and referrals to support individuals and groups to reduce the harms of substance use through case management.
  • Provide information and education to YISSA residents about harm minimisation and treatment options. Provide AOD support and advice to staff within the IHSAP program where required.
  • Build and maintain strong networks that will ensure best practice and benefit people accessing support for AOD issues.
  • Display a culturally appropriate, strength based, client centered attitude with consideration to CAAPS values.
  • Ensure accurate and timely recording of client information as per IHSAP procedures.
  • Concerns about the safety and wellbeing of children and young people are identified and responded to effectively.

Your skills, experience and qualifications include: 

  • Demonstrated experience working with people with substance use issues including provision of case management services, group program facilitation and advocacy.
  • Recognised qualifications in AOD or Community Services.
  • Ability to support people with complex needs through AOD Brief Interventions and information and education to reduce harm from AOD use.
  • Strong networks that are relevant to people experiencing AOD issues.
  • Ability to build therapeutic relationships with people experiencing AOD issues.
  • Demonstrated ability to work positively as part of a multidisciplinary team.
  • Demonstrated understanding of current issues impacting the delivery of AOD treatment for individuals and families affected by substance misuse.
  • A sound knowledge of current local and national strategies related to the AOD field.
  • Current First Aid Certificate
  • Current NT Driver’s License (minimum C Class)
  • Current Criminal History Check
  • Current Ochre Card (Category E)
  • Experience working with Aboriginal people with substance use issues including delivering community services.
  • Experience in effectively to Aboriginal and/or Torres Strait Islander people.
  • Demonstrated computer skills including MS Suite, email, and records management.
