headspace is Australia’s National Youth Mental Health Foundation. Our mission is to deliver improvements in the mental health, social wellbeing and economic participation of young people aged 12 to 25 years. Our Strathpine centre was established in 2020 and has been warmly received within the community since inception.

 

Our centre offers support for young people through individual mental health interventions, vocational support, AOD counselling and groups. In addition, our team hold collaborative relationships within the community maintained by co-location of services such as Neami National and regular attendance at events such as the Pine Rivers Community Network Meeting.

 

The opportunity:

The Youth and Family Support Coordinator is an exciting new role at headspace Strathpine that is responsible for coordinating the implementation of key parts of the young person’s care plan. This is a maximum term full-time opportunity within an established centre filled with a welcoming multidisciplinary team that fosters an environment that prioritises wellbeing and balance.

 

As a Youth and Family Support Coordinator no two days will be the same as you will deliver psychosocial interventions (including skills development, improving self-care, and increasing social connection and engagement in structured activities) and ensure that there is effective coordination among the stakeholders supporting the young person.

 

Responsibilities include:

  • Collaboratively develop and manage care plans for young people to address their mental health and psychosocial needs.
  • Coordinate and monitor the young person’s care in collaboration with internal and external stakeholders.
  • Provide individualised psychosocial support and interventions to young people, including skill development and increasing social connection.
  • Incorporate family-inclusive practices and collaborate with family members and relevant individuals to support the young person’s mental health.
  • Maintain strong relationships with external stakeholders, conduct risk assessments, and ensure high-quality client records while complying with professional standards and legislative requirements.

 

About you:

You will need to be skilled in coordinating young people’s needs, including organising psychosocial interventions, care plan management, risk assessment, working with diverse populations, and collaboration

  • Tertiary qualifications and/or relevant experience in youth work, care coordination, support work, or related fields.
  • Experience working with young people’s mental health and well-being, delivering youth-friendly, inclusive, and high-quality services with a recovery and strengths-based approach.
  • Experience developing, managing, and implementing care plans for clients with mental health difficulties.
  • Strong organisational and time management skills with the ability to work independently.
  • Excellent written communication skills for professional documentation, including case notes.
  • Highly developed interpersonal communication and decision-making skills, involving staff, clients, and their families in service delivery.
  • Ability to work effectively with multidisciplinary teams and collaborate with various external stakeholders in health, mental health, and psychosocial services for young people.
  • Knowledge of local support services and networks for referrals and additional support for young people.
  • Familiarity with mental health system, government policies, legislation, and professional standards relevant to working with young people and their mental health.
  • Experience in conducting risk assessments, developing and implementing risk management plans, including crisis management.
  • Strong advocacy skills, particularly when liaising with external entities such as schools and medical services.
  • Community knowledge and ability to establish a presence in the community to facilitate optimal outcomes for young people.
  • Passion for fostering and promoting independence.
  • Autonomy and ability to work both independently and in teams.
