Case Coordinator – Orange Housing Plus

JOB DESCRIPTION

Housing Plus is a leading NFP Community Services Provider with a 40 year history, providing housing, domestic violence, homelessness and other community services in NSW. We are currently looking for a Case Coordinator to join our Initial Transitional Service (ITS) team based in Orange.

The ITS program is aimed at supporting individuals with complex needs (such as mental health, acquired brain injury, alcohol and other drugs) upon initial exit from custody. The program works with individuals for a period of up to 12 weeks during the critical transitional period to support the reintegration into local community through linking with identified services, building capacity in the individual and developing a network of supports.

Working in close partnership with a Corrective Services NSW Case Manager and engaging with other service providers and partners, the ITS Case Coordinator will be responsible for the day-to-day implementation of a case plan to provide collaborative service delivery to support parolees.

To be successful, you will ideally have relevant experience in working within the corrective services, disability, acquired brain injury, mental health and/or alcohol and other drug sectors, with demonstrated understanding of best practice approaches in dealing with disadvantaged members of the community and proven ability to professionally and sensitively deliver client services. The ability to work with and understand issues affecting Aboriginal clients and clients from a CALD background is essential.

Relevant qualifications in community services or similar will be highly regarded.

You will need a Current Driver’s Licence and willingness to obtain a Police Check and Working with Children Check.
