About the role

 

The Family Support Worker will provide social and emotional support and health education to women and family members throughout the woman’s pregnancy and up to three years following the birth. The key functions of this role are to build culturally safe relationships, provide education, offer support, and provide appropriate referrals.

  • Identification of families who require additional support and provide internal/external referrals.
  • Develop and action in-depth holistic family case plans to address support needs.
  • Contribute to the coordination of relevant parenting, cultural, hobby, and educational classes through the Jajumbora Child and Family Centre.   
  • Deliver evidence-based and culturally appropriate clinical, health promotion, and educational services to clients and their families.
  • Support and advocate for the appropriate clinical care of mothers, babies, and the wider family group throughout pregnancy, post-natal care, and through early child development (including 715 Health Checks).  
  • Assist with transporting clients under the direction of the Manager.
  • Assist and collaborate with the Administration team to ensure the smooth and efficient running of the hub as directed by the Manager.

Duties

1.Provision of Social Emotional Health Support.

2.Provision of Health promotion and parenting education and support.

3.Facilitating the Logan BiOC Hub client journey.

4.Partnering with BiOC Stakeholders.

5.Commit to a high level of quality management for ATSICHS through use of LogiQC quality management systems.

6. Commit to ongoing continuous quality improvement processes with annual reviews into the efficiency of the service provision.

7. Comply with ATSICHS policy and procedure in addition to other legislation, guidelines and regulation.

  • Base salary starts from $64,263 to $70,043 per annum plus 11% superannuation.

COVID-19 Requirements

  • Immunisation history showing you have received two doses of a Covid 19 vaccine.

Required

  • Demonstrated ability and experience in working with Aboriginal and Torres Strait Islander communities and their leaders, respecting traditional culture, values, and ways of doing business.
  • Ability to effectively identify psychosocial factors influencing pregnancy and parenting and provide support and/or referral to specialist services using a strengths-based approach.
  • Ability to contribute effectively to the Birthing in Our Community clinical and operational performance through provision of evidence-based care and information, and adhering to relevant workplace instructions, policies, and procedures.
  • Demonstrated organisation skills, including an ability to multi-task, plan, set and prioritise workloads to meet deadlines, and to work both independently and collaboratively to achieve outcomes.
  • Current Blue Card (Essential prior to commencement).
  • Current National Police Check.
  • Current C Class Drivers Licence (Qld) and the ability to travel daily around SEQ.

Preferred

  • Minimum Diploma level qualification in Human Services, Community Services, Psychology, Social Work, or related field.
  • Experience in Case Management, specifically in providing supports to at-risk families/youth.
  • Experience in a clinic setting and an understanding of the cycle of care patient system.
  • Experience working one on one with a medical professional to achieve program outcomes.
