ABOUT THE ROLE
We are currently recruiting for an experienced Social Worker/Discharge Planner at Holroyd Private Hospital, who can work part-time. The hospital provides Inpatient and outpatient Rehabilitation services to clients with a wide range of health issues in Orthopedics, Cardiac, Pulmonary, Neurological, and geriatric populations.
We are looking for someone who has…
- A bachelor’s or post-graduate degree in social work
- Excellent negotiation, decision-making, and advocacy skills including collaborative stakeholder consultation.
- Knowledge, skills, and clinical experience in Social Work clinical practices.
- The ability to work as a member of a multidisciplinary team.
- Good time management skills and ability to prioritize workload.
Your duties will include the following:
- Proactive Discharge planning including arranging services
- Liaising with community-based services
- Liaison with families in regard to discharge planning and assisting with Aged care placements process.
