This is an exciting opportunity to join our dynamic Enhanced Support Team. As part of a multi-disciplinary team you will provide case management services to eligible NDIS participants and others with a focus on restoring, improving, or maintaining independent functioning and wellbeing. You will have a range of case management referrals across life span.
Within a reablement model you will work with participants to provide education, introduce techniques for self-management, and provide advice and supervision for improvement in people’s capacity.
Selection Criteria:
- A Bachelors Degree in Social Work – new Graduates or those waiting to graduate are welcome to apply.
- Membership of or ability to acquire membership of the Australian Association of Social Work.
- Demonstrated experience in complex case management and positive behaviour support.
- A sound knowledge of NDIS (desirable).
- Current Driver’s Licence.
