JOB DESCRIPTION

Are you passionate about providing support to children, young people and their families to support long term positive outcomes?

Exciting opportunity for an experienced Family Reconnection Coordinator! All Care Australia is experiencing an exciting period of growth and change, positioning the company as an emerging leader within the human services sector. All Care Australia is aiming for organic growth, diversification, and national expansion.

About us:

All Care Australia provides therapeutic home environments for children and young people with histories of complex trauma, abuse, and neglect. We care deeply about the well-being of vulnerable children and young people and aim to make a positive difference in their lives.

All Care Australia uses a trauma-informed approach when supporting the young people in our care. Our goal is to provide the young people with a sense of family, support to heal, a chance to positively connect with others and the opportunity to rebuild their lives.

We deliver services aligned with our values and our practice framework.

Our values are:

  • Respect: we see people’s value and co-create opportunities for a bright future.
  • Integrity: we show our uncompromising ethical principles and values.
  • Safety: we work with people to carefully mitigate risk while living a full life.
  • Excellence: we use continual improvement to strive for best practice.

About the role:

The Family Reconnection Coordinator will work alongside children, young people and their families where there are identified safety concerns and ongoing statutory intervention. The Family Reconnection Coordinator will provide psychoeducation and practical support to address existing worries, with the intention of increasing the level of safety within the family home.

By addressing identified concerns we aim to stabilise the family home and reduce the risk of an Out of Home Care arrangement. In the event there is a current Out of Home Care arrangement we will to work directly with the child/ young person, their family and relevant stakeholders to support reunification without the ongoing need for statutory intervention.

Where there are barriers to reunification, the Family Reconnection Coordinator will be required to work with outreach services, support staff, kinship, kith, and foster carers, to increase their capacity to prevent placement breakdowns and stabilise their Out of Home Care Placement.

Key Responsibilities:

The Family Reconnection Coordinator will:

  • Work collaboratively with families, their support networks and relevant stakeholders to ensure identified concerns are addressed therefore promoting positive outcomes for children, young people and their families.
  • Assess family needs via active engagement, observations, and the review of information provided such as family history and child safety case plans.
  • Utilise relationship based, trauma-informed, and attachment-based theories to provide psychoeducation and practical support to families and relevant stakeholders, therefore promoting a positive family environment and healthy child development.
  • Ensure all documentation, assessment, and reports are completed within a timely manner.
  • Develop strong relationships with relevant stakeholders and partners within the sector.

Essential Criteria:

  • At least 2 years’ experience in a similar role with a solid understanding of child protection legislation.
  • Tertiary qualification at diploma level or above in Social Work, Psychology or relevant discipline.
