Transitional Support Worker

  • Full-time, Fixed-Term until 30th June 2024
  • Location: Hamilton North and Bateau Bay
  • This role is paid under the Social, Community, Home Care & Disability Services Award 2010- Level 5 

 

The Opportunity: 

The Premiers Youth Initiative (PYI) is an initiative that aims to prevent homelessness among young people leaving care by diverting them from the homelessness service system before they become homeless or at risk. 

The role of Transitional Support Worker will be to provide transitional support to young people in targeted locations to build their capacity to establish and maintain long term accommodation with a particular emphasis on building capacity to establish and maintain share accommodation over time through up to 4 years transitional support.

The Essentials:

  • Tertiary Certificate in Youth Work/Community Welfare/Social Work 
  • Excellent knowledge, understanding and experience of working from a trauma informed, strengths-based approach with young people who have lived in Out of Home Care
  • Experience providing incidental counselling and mentoring to young people with complex needs
  • Experience in building relationships, networking, working cooperatively with, referring to and advocating with a range of diverse services that offer supports to young people
  • Well-developed knowledge of diverse cultures and community needs and issues as they relate to care of children and young person’s so as to effectively manage reports of risk of harm to children and young people
  • Experience and highly developed skills in goal setting and action planning for young people leaving Out of Home Care
  • Knowledge and understanding of the rights, legislation and resources available to children and young people who have been placed in Out of Home Care, including requirements for leaving and aftercare planning
  • Ability to utilise confidentiality principles, prioritise competing demands and meet deadlines while working independently
  • High level of planning and organizing skills to support allocated clients, together with investigative, assessment, problem solving and analytical skills.

The successful applicant will possess or willingness to obtain the following: NSW Working with Children Check, NDIS Worker Screening Check, first aid & CPR certificate and undertake a pre-employment functional medical.
