Psychosocial Support Coordinator
Multiple positions available

Permanent full time 80 hours (including ADO)
Part time available on request.

Multiple locations available

About the role
The Psychosocial Support Worker will work collaboratively with the multidisciplinary team, which includes clinical staff and peer workers, assisting participants to navigate services, offer linkage and referral, provide practical support when required, offer support to carers and family and develop and assist with safety planning.

  • Work with consumers to provide a 12 week program with a recovery process
  • Work collaboratively with the HOPE Team and other external and internal agencies
  • Review safety plans and establish suicide risk with consumers
  • Provide knowledge and understanding of recovery programs

Key Responsibilities

  • Engage with consumers and work collaboratively with them to achieve a recovery model
  • Documentation as required
  • KPI completion as required by the team and the health service
  • Develop relationships with other community services and mental health services
  • Provision of services in line with the HOPE model of care. 

Skills and experience

At least one of the following education standards or level of practical experience:

  • Diploma level qualification in a relevant field including but not limited to: Alcohol and other drugs; Community Service; Disability; Mental Health; Youth Work or similar.
  • Students currently completing university degrees in nursing, social sciences or allied health including: Occupational Therapy; Psychology; Social Work or similar.
  • A minimum of 2 years full-time experience in working with and providing support to persons experiencing psychological distress; mental ill health and/or suicidal ideation.
  • Experience working with community members who have psychosocial problems.
  • Ability to work as a member of a team
  • Ability to ensure accurate documentation
