JOB DESCRIPTION

  • Provide outreach wellbeing support to participants with mental illness across the SE region
  • Full-time & Part time roles available – contracts available ending 30/6/2025 with potential extension
  • SCHADS Level 4 – $41.52 – $44.68 p/h. Salary packaging options and flexible employment conditions available

 

About You
Are you a Wellbeing mentor and/or have a lived experience of mental illness and a passion to use this experience in supporting others to create connections and links within their community?
Or do you have a lived experience as a Carer of someone and have the skills to work from a family inclusive way? 
Are you able to implement a variety of therapeutic practices to cater to the needs of individuals requiring psychosocial support?

Additional attributes we are looking for include: 

  • Ability to work autonomously to identify individual and family needs
  • Effective listening skills & an adaptable communication style
  • Capable of advocating for others and building appropriate support networks 
  • Ability to contribute positively within a team, sharing ideas and working collaboratively
  • Ability to source suitable and sustainable supports and services for participants
  • Prior lived or living experience supporting people with dual diagnosis or dual disability

It is a requirement of this position to be fully vaccinated against COVID-19 and meet all compliance requirements in relation to NDIS Work Screening, Working With Children’s Check, work rights and possess a current & valid driver’s license.
About the Role
You will oversee a caseload of consumers and work collaboratively within your team using the recovery model. Our goal is to enable rather than do, to promote independence, development and confidence of the individuals.  

We take appropriate actions to ensure that our staff and the people we support are as safe as possible. You will be expected to adhere to all OHS standards, requirements and processes, including (but not limited to) wearing appropriate PPE and regular hygiene and cleanliness practices. 

The role entails: 
 

  • Providing case management, assertive outreach and group facilitation to support participants and their families in their own home and within their community 
  • Providing short term assistance to enable participants to work towards their goals
  • Increasing self-management and independence for participants
  • Developing and maintaining professional relationships with participants and all relevant stakeholders 
  • Providing accurate and timely case notes including risk management and recovery plans 

It is a requirement of this position to meet all compliance requirements concerning NDIS Work Screening, Working With Children’s Check, work rights and possess a current & valid driver’s licence.
